Injury While Playing in AISL

If you get injured playing at the dome in the AISL, please review the Player insurance documents website.

How to claim

1. Complete page 1 of the claim form
2. Complete and submit the form below to describe the injury.
3. You will be sent a completed and signed copy of page 2 of the claim form
4. Have attending physician complete pages 3-5 (the cost is your burden).
5. You submit pages 1-5 (your claim documents) as per method described on the first page of the claim document.
